Wellbeing self-assessment

Ten questions, about two minutes. A structured way to reflect on how the past couple of weeks have actually been — not a diagnosis, and nothing is stored.

Question 1 of 10 0%

This self-assessment is for awareness and self-reflection only. It is not a diagnostic tool and does not replace professional evaluation. It does not diagnose or label any mental health condition. Your answers stay in your browser: nothing is saved, sent, or recorded, and we do not ask for your name, email, or any other personal information to use it.
How to read your result

A prompt for reflection, not a verdict

Each question is scored from 0 (never) to 4 (very often), giving a total between 0 and 40. A higher total simply means more of these experiences have been present more often over the past two weeks. It is not a severity rating, and it corresponds to no diagnostic category.

What tends to be more useful than the number is the pattern: which specific areas you marked as happening often or very often, and whether they cluster around sleep, mood, concentration or connection.

If your responses suggest things are difficult, the most reliable next step is a conversation with someone qualified. If they suggest things are steady, that is worth knowing too.
